Enhanced Collaboration between the Day Hospital and General Practitioners
Based on the observation that patients suffering from chronic diseases should benefit from continuous care at home and thanks to regular awareness-raising by the medical profession, we have put in place a follow-up, thanks to the day hospital and better collaboration with general practitioners, allowing particular attention to the development of comorbidities and the deterioration of the disease.

In addition to current Self-Management Education programs and themed days, the day hospital allows patients to find themselves in a medical environment to benefit from a general reminder of the management of their disease as well as individualized follow-up to ensure the continuation of their treatment under the best conditions.
These days allow patients to benefit from the high-quality care provided by our medical and paramedical teams. We will also be able to welcome patients with specific pathologies related to the care of elderly individuals with multiple health conditions, as well as patients suffering from chronic diseases (Alzheimer's, urinary rehabilitation, diabetic foot, geriatric patient education, mental health, obesity, diabetes).
